New Contraindications

Enoxaparin sodium (Clexane and Clexane Forte) is now contraindicated with immune mediated heparin induced thrombocytopenia (HIT) within the past 100 days or in the presence of circulating antibodies.

Erythromycin (Eryc Capsules, Mayne Pharma Erythromycin) is now contraindicated with terfenadine or astemizole. Do not use erythromycin concomitantly with HMG-CoA reductase inhibitors (statins) that are extensively metabolized by CYP3A4 (lovastatin or simvastatin), due to the increased risk of myopathy, including rhabdomyolysis.
